Abstract
Objectives: This study aims to compare between same-session ERCP and laparoscopic cholecystectomy and ERCP followed by delayed laparoscopic cholecystectomy in patients with cholecystocholedocholithiasis as regard to incidence and severity of acute pancreatitis. Background: Management of common bile duct (CBD) stones is very important to avoid serious complications. ERCP followed by laparoscopic cholecystectomy (LC) is the best treatment option nowadays. The time of LC post ERCP is controversial and depends mainly on technical facilities and the experience of the surgeon. Performing the two interventions at the same session proved to be a good option for the patient. Methods: Forty patients with cholecystocholedocholithiasis scheduled for ERCP and LC were included. The patients were divided into two groups, same session group and delayed group. Results: There was a statistically significant increase in group II (delayed) compared to group I (same session) as regard to operative time, hospital stay, CBD diameter and laboratory results of alkaline phosphatase and gamma-glutamyl transferase (GGT). There was no statistically significant difference between the two studied groups as regard to incidence of acute pancreatitis, other complications, age, sex, bilirubin levels. Conclusion: Our study suggests that performing ERCP and LC at the same session showed no difference in the incidence of acute pancreatitis from the delayed group, but same session intervention is a good option as it proved to be better as regard to operative time, exposure times to anesthesia and hospital stay.
