Abstract

Objectives: to study the relationship between anti TNF and thyroid functions in patients with rheumatological diseases

Background: Anti Tumour Necrosis factor (anti TNF) are important drug used for treatment of many autoimmune diseases. Thyroid functions may be affected by anti TNF therapy.

Methods: a prospective cohort study conducted on 50 patients diagnosed with rheumatological diseases and were candidated to receive their first dose of anti TNF at rheumatology unit at Menoufia university hospital at the period between October 2022 to March 2024. The studied patients divided into two groups:- Patients with rheumatological diseases and normal thyroid functions as group 1 and Patients with rheumatological diseases and autoimmune thyroid diseases as group 2. All patients subjected to detailed history taking, complete physical examination ,calculation of disease activity score and laboratory investigations including TSH ,free T3 , free T4 , TRAB and anti TPO.

Results: TSH level and anti TPO level decreased significantly after anti TNF treatment ,While no significant change of FT3 level after anti TNF treatment in both groups was observed . FT4 level increased significantly after anti TNF treatment in group 1

Conclusion: Our study showed that use of anti TNF therapy is not associated with development of thyroid diseases in euthyroid patients, moreover there was significant improvement of thyroid functions tests in hypothyroid patients with rheumatological diseases after anti TNF therapy
