Objectives: This study aims to compare the short-term outcome of morbidly obese patients treated with one-anastomosis gastric bypass (OAGB) with those treated with single-anastomosis sleeve ileal (SASI) operation.

Background: OAGB and SASI are two new operations of the mini-gastric bypass that have been commonly used in the treatment of obesity recently.

Patients and methods: This prospective controlled study was conducted at the General Surgery Department, Menoufia University, and it included 40 patients divided randomly into two groups: group (A) included 20 morbidly obese patients who were treated with OAGB, and group B included 20 morbidly obese patients treated with SASI.

Conclusion: Both OAGB and SASI are safe and effective in the treatment of obesity, but OAGB had a greater effect on weight loss and improvement of comorbidities with lower nutrient deficiency problems.

Results: A total of 40 patients were included in our study (20 in OAGB and 20 in SASI). OAGB conferred significantly higher percentage of excess weight loss at 12 months than SASI (88 vs. 60% with P < 0.001). Improvement in diabetes mellitus and hypertension after OAGB was better than SASI (100 vs. 42% with P = 0.038 and 85 vs.60% with P=0.252, respectively). OABG required longer operation time than SASI (164 vs. 156 min with P= 0.161). SASI caused more nutritional deficiencies than OAGB.
